Proof of Insurance

HMS ensures that all vendors that our clients work with are insured.  Include with your invoice a Certificate of Insurance that includes both Liability and Workers' Compensation coverage, and has the Certificate Holder listed as:

HMS Inc.
P.O. Box 2458
Alpharetta, GA 30023-2458

If you do cannot provide this proof of insurance, then you will need to complete HMS' insurance waiver form.


Form W-9

To complete your payment, HMS will also need to have a Form W-9 from the IRS on file.  If you have not worked with HMS in the past, please be sure to submit this form with your invoice.

To help us get your payment to you as soon as possible, send you invoice to HMS by emial to Invoices@hms-inc.net.

If you cannot submit your invoice by email, then send it by fax to:

Please submit your Invoice only one time.  Submitting your Invoice by both email and mail may delay your payment.
